Treatment is not advised in the presence of untreated caries, active periodontal disease, significant bone loss, severe malocclusions or pronounced skeletal discrepancies. It may also be contraindicated if the patient is unable or unwilling to wear the aligners for the required daily period. In such cases we recommend alternative orthodontic options or first address the underlying dental and periodontal conditions.

In most cases it can, although treatment planning requires particular care. Conventional crowns and bridges usually do not prevent aligner therapy. In the presence of implants, the implant supported teeth themselves cannot be moved, therefore the movement of the neighbouring teeth must be planned in relation to these fixed points. Every such situation is preceded by detailed diagnostics and careful planning.

In this situation you should always contact the clinic. Depending on the stage of the treatment, it may be appropriate to return temporarily to the previous aligner, to move forward to the next aligner, or to order a replacement. It is important to avoid leaving the teeth without an aligner for an extended period, as this may affect both the precision and the timing of the treatment.

If the daily wear time is reduced, tooth movement will slow down. If the aligners are regularly worn less than recommended, the teeth may not reach the planned positions, the next aligner may fit poorly, the overall treatment time may increase, and in some cases the treatment plan may need to be revised. Occasional minor deviations can usually be managed, but in case of repeated underuse it is important to discuss the situation with the treating orthodontist.

At the beginning of treatment a slight change in speech or articulation can occur, but most patients adapt fully within a few days. Office work, customer facing roles and even speaking intensive professions can usually be continued without significant limitation. If a patient’s profession is particularly sensitive to speech quality, this is discussed in detail during treatment planning.

Aligner therapy can be carried out in many patients with bruxism, but it requires a personalised approach. Increased functional load may place higher stress on the aligner material, therefore more frequent reviews and a carefully planned retention protocol may be necessary. In some situations an additional protective or night time appliance can also be indicated, which is decided by the treating clinician.

The frequency of review visits depends on the individual treatment plan and the level of patient cooperation. In general, appointments are scheduled every few weeks, sometimes at slightly longer intervals. During these visits we monitor the progression of tooth movement, fine tune the plan if necessary and address any practical questions the patient may have.

The duration of retention is individual, but as a general principle, stabilising the result demands at least as much attention as the active phase itself. Immediately after treatment a retainer is usually worn on a daily basis, after which the wear time can gradually be reduced. The exact protocol is always defined on the basis of the initial situation and the extent of tooth movement that has been carried out.
